Model QDROs & QDRO Procedures - 401(k)

On this page you will find model QDROs and QDRO Procedures to assist you in preparing a QDRO for UPS 401(k) and Savings Plans.

Prior to filing any documents with the court, parties are strongly encouraged to submit a draft QDRO to UPS for preapproval. You may submit a draft by going to the Home Page and clicking on "Submit a QDRO".

Please note that once a model QDRO or QDRO Procedures are downloaded for any of these plans, an administrative hold will be placed on the Participant's account.
